Consumer Directed Community Supports (CDCS) is a flexible service option available under MN DHS Waivers, including CAC, CADI, DD, EW, and BI waivers, that gives persons more flexibility and responsibility for directing their services and supports, including hiring and managing direct care staff.
All SWCIL Support Planners are Certified by the MN Department of Human Services.

To assist you with developing support plans and budgets under CDCS, the Southwestern Center for Independent Living offers Support Planner Services which include, but are not limited to:
· Help develop, monitor and revise the community support plan and budget · Find and connect persons to both licensed and non-licensed providers · Promote and enhance the self-determination process · Facilitate community access and inclusion · Assist persons to hire, support and/or train their employees For more information on CDCS such as the process for application and eligibility, please click here. |

Consumer Success Story...
After hearing about SWCIL services repeatedly as they engaged in parent-teacher conferences while their son Reggie was in high school, the Knutsons were confronted by questions regarding their son's future. Once connected with SWCIL, their first project was to apply for a Medical Assistance waiver and Supplemental Security Income for their son. After securing this, they worked with SWCIL to establish Consumer Directed Community Supports (CDCS) and to write a Community Support Plan and Health and Safety Plan for their son, who is now doing well as he receives the meaningful support of these various resources. |
